I'm not really sure. Sometimes it's by eating rotten foods, interacting with roaches, an extremely off-the-chart unclean home, etc. I usually just make my sims take a bubble bath (I'm not sure if this really has any effect but it works!), sleep, relax, nap (whatever-sleep best tho) then eat a filling meal. This should work. Just don't keep them up too long.

Yup roaches....imparticularly having your rubbish bin kicked over. Keep rubbish picked up..... and it spreads through your entire community with contact between the Sims so quarantine any that are sick. There are various 3rd party patches available to get around it.
